I hope you and your family are doing well. It is with mixed emotions that I am writing to let you know that I will be closing my family practice and relocating from Victoria. My last day of practice will be September 15, 2026.

This has been a very difficult decision for me, and one that I have considered carefully. While I am excited to begin a new chapter closer to my family, I am saddened to be leaving the patients and community that I have had the privilege of caring for over the years. I understand that this news may be disappointing and that finding a new family physician can be challenging. I sincerely apologize for any stress or inconvenience this transition may cause, and encourage you to begin looking for a new primary care provider as soon as possible. Although I have explored options for transferring my patient panel, there is currently no physician at Doctors Medical Clinic who is able to assume care of my patients

Until September 15, 2026, I will continue to provide your medical care, including reviewing investigation results, managing ongoing health concerns, and renewing prescriptions as appropriate. To avoid interruptions in your care, please ensure that any prescription refill requests are submitted before my practice closes. Following the closure of my practice, any referrals, specialist consultations, laboratory investigations, and diagnostic imaging requests already in progress will remain active. However, healthcare providers involved in your care should be advised that I will no longer be available to receive reports or provide follow-up. Once you have secured a new family physician, please arrange to have future reports directed to them. In the interim, walk-in clinics or Urgent and Primary Care Centres may be able to assist with ongoing follow-up needs.

To ensure continued access to your medical information when my practice closes on 15 September, your records will be securely stored and managed by MedRecords, an independent non-profit organization specializing in medical record management. You may request a copy of your records or arrange for their transfer to another healthcare provider directly through MedRecords for a modest administrative fee.

Please note that Doctors Medical Clinic will only process record requests from your new family doctor or nurse practitioner until 15 September. After 15 September, all such requests need to be made to MedRecords.
